Activities Approval Form

Before any activity that requires a permit holder can take place Leaders must ensure that all of the relevant requirements of section 9 of P.O.R have been complied with and that the activity has been approved by the District Commissioner (see P.O.R Rule 9.1.2) or their nominee.

The nominees for Glendon are DDC Group Support and DDC Programme. By submission of this form, you confirm that all Risk Assessments are in place and that they have been read and agreed to by all leaders/adults attending the event.
